Download a MAP Superannuation Plan Product Disclosure Statement (PDS), including any Supplementary Product Disclosure Statements.

Ensure you read the information carefully and fill out the Application Form located in the back of the MAP Superannuation PDS.
Return the completed MAP Superannuation Application Form to MAP at:
